Medical Records are managed at the clinic level. Please follow the instructions below to request records from your clinic.
Medical records from any state except Minnesota
The clinic-specific Release of Information form can be requested from your clinic by email. Each clinic email address can be found in the “contact us” section of the clinic location page. If your clinic has recently closed, please use the instructions later on this page.
Medical records within Minnesota
Minnesota clinics are now owned and operated by Sagent Behavioral Health. Please visit their Medical Records Request page to find their records request process.
Medical records for a closed clinic
If your clinic has closed, please complete the Release of Information (ROI) form and:
